Complex Case Manager

The Role:

We are looking for a passionate and skilled Complex Case Manager (CCM) to partner in our mission to provide high-quality long-term mental health care to clients who need it the most. The Complex Case Manager will be responsible for providing professional case management and supportive services to high-acuity or actively suicidal clients to assist them with crisis stabilization and acute symptom reduction. Our ideal candidate is experienced, emotionally intelligent, capable of effectively navigating others through stressful situations, and able to work independently with an appropriate level of guidance.

Who you are:

  • 2+ years of experience providing professional case management services to clients with chronic and/or severe medical or mental health conditions
  • Masters degree in social work, psychology, counseling or related field
  • Possess strong technology skills
  • An excellent communicator, both verbal and written, with exceptional organizational skills
  • Self-driven, adaptable, and solution-oriented
  • Comfortable with navigating nuance and pioneering new programs or initiatives
  • Familiarity with NQCA care management practices preferred
  • CCM (Certified Case Manager) would be a plus, CCM-eligible preferred
  • You bring a positive attitude, and will cultivate a culture of trust, advocacy, and growth

How your skills and passion will come to life at Cerebral:

  • Manage a revolving caseload of high acuity clients 
  • Conduct ongoing assessments, care planning, skill-building, goal management, and psychoeducation 
  • Lead 30 minute intake sessions with clients via phone or video 
  • Lead weekly 15 minute sessions with clients via phone
  • Provide live crisis de-escalation for CCM clients in crisis; work with emergency contacts and resources as required
  • Lead care coordination initiatives with client’s Cerebral care team members, family and other social supports, as well as other healthcare providers as needed
  • Own community resource research, identification, and brokering to support clients with addressing social determinants of health 
  • Communicate to clients via text message, email, and/or the EMR chat feature
  • Review possible crises flagged by our Crisis Message Detection (CMD) system and complete crisis response protocol when needed
