About the Role:
We are hiring a Clinical Pharmacist to help our members on their journey to healthier hearts. In particular, the Clinical Pharmacist will help to identify and inform users about prescription medication treatment gaps, and empower them to speak with their prescribing provider to close gaps that exist. In this role, you will evaluate patient charts, identify prescription medication treatment gaps, and then complete outreach to the patient after determining if there exists a prescription medication treatment gap that should be closed. This is a full-time, remote position within our company.
Responsibilities:
- Review patient medical records to determine if guidelines-based prescription medication gaps exist.
- Communicate with patients about prescription medications gaps that should be closed.
- Collaborate with healthcare providers (e.g., physicians, nurses) to recommend medication therapies based on clinical guidelines and patient-specific data.
- Maintain detailed records of all consultations and communications with patients, including up-to-date electronic health records (EHRs).
- Stay informed on latest healthcare practices, telehealth technologies, and continuing education requirements.
- Help connect patients and family members to community resources, support groups, or other services they may need.
Qualifications:
- Doctor of Pharmacy (Pharm.D.) from an accredited school of pharmacy, preferably with Board Certification in Pharmacotherapy (BCPS) or Ambulatory Care (BCACP).
- Minimum of 2+ years of clinical pharmacy experience, preferably within telehealth.
- Actively maintains multi-state pharmacist licensure.
- Strong knowledge of pharmacology, clinical guidelines, and best practices in medication therapy management.
- Familiarity with virtual patient care platforms and protocols.
- Understanding of patient data privacy laws (e.g., HIPAA) and secure handling of electronic medical records.
- Ability to work independently and collaboratively, as required, in a fast-paced, start-up environment.
The US base salary range for this full-time position is $110,000 to $160,000. Salary ranges are determined by role and level. Compensation is determined by additional factors, including job-related skills, experience, and relevant education or training.
